The limitations of BMI: when the result can mislead
Understand why BMI does not directly measure body fat and which people require more careful interpretation.
BMI is useful because it is simple and standardized. That simplicity is also its main limitation: it uses only weight and height.
BMI does not directly measure:
- body-fat percentage;
- muscle mass;
- bone mass;
- fat distribution;
- fitness or metabolic health.
It can classify a muscular athlete as overweight despite relatively low body fat. In an older adult with low muscle mass, it may understate concerns. Current pregnancy weight should not be read through standard categories, while children and teenagers require percentiles by age and sex.
Interpretation can also vary among populations because the relationship between BMI, body fat and health risk is not identical for everyone. This does not make BMI useless; it means a screening result should lead to context, not a verdict.
Useful complementary measures may include waist circumference, body-composition estimates, blood pressure, glucose and lipid tests, fitness, weight history and a clinical assessment. Each has its own limitations.
Use the BMI calculator as a consistent starting point. Do not use it alone to diagnose a condition or choose an aggressive weight target.
